As a member of our private
Health Care Association we would love to meet you in person and go over
all of our programs and processes if it is possible right here in our
office in Oklahoma City, Oklahoma.
If not, there is never a need for you to come to our office. We can evaluate and treat you long distance from anywhere you are at in the
world and mail you your products needed. For more information please
feel free to
call or write for additional information.
This is a private Healthcare Association, you must join the association to be able to get any type of service at our Oklahoma City office, or to buy any product from Back to Eden. We no longer serve the public in any way, so you must fill out the forms, sign them and mail the originals to our office to be processed and approved before any business can be conducted with this association. The forms can be found below and printed out.
All documents, photos, and paperwork sent to Back to Eden, becomes the property of Back to Eden and will not be returned to the sender.
The fee is 10.00 USD per year.
